How Does Electromyography Work?

Electromyography is built on a simple principle: Electrical activity of healthy muscles at rest is next to nothing, while electrical activity of contracting muscles is significantly higher. A trained physician reads those changes like a map of what is happening beneath the skin.

During an EMG test, a thin needle electrode is inserted into the target muscle. That electrode picks up the muscle’s electrical activity both at rest and during a light contraction, and the signal is displayed on a monitor as a waveform while also being audible through a speaker.

EMG testing is almost always paired with a nerve conduction study, sometimes called an NCV test, during the same visit. Where the EMG measures the muscle’s response, the nerve conduction test measures how quickly and how strongly an electrical signal travels along the nerve itself using small electrode stickers placed on the skin. Together, these two tests give a complete picture: the nerve conduction study identifies where along the nerve pathway a problem may exist, and the EMG confirms how that problem affects the muscle the nerve controls.

Continue taking your regular medications unless your physician tells you otherwise, and let our staff know if you take blood thinners, since this can occasionally affect how the test is performed.

During the appointment, the nerve conduction portion happens first. Small electrode stickers are placed on the skin over specific nerves, and a mild electrical pulse is sent through them to measure how quickly the signal travels. Patients typically describe this sensation as a brief, quick tap rather than significant pain. 

The EMG portion follows, wherein a thin needle electrode is inserted into the muscle being studied. You may feel a brief pinch during insertion and some mild discomfort while the muscle contraction is elicited, but the sensation is generally well-tolerated and passes quickly once the electrode is repositioned or removed.

The full appointment, including both the nerve conduction study and the EMG, generally takes under an hour, depending on how many muscles and nerves need to be evaluated. Mild soreness at the needle sites is common afterward and usually resolves within a day or two.
